​1. Navigate to https://www.pixifi.com/admin/

2. Templates > Questionnaires

3. Click "Questionnaire Templates"

4. Choose one of our pre-made Templates from our Questionnaire wizard or create a new one from scratch by selecting [+ Create New]

5. When you create a new Template - give it a name and save

6. Click "Yes"

7. Click "Add Question"

8. Now all the possible field types will show up in a pop-up window - select the ones you need to add for your Questionaire Template

9. Click "Save"
